Personally I do with food since stomach upset can be a side effect.

Grapefruit juice interferes with gut enzymes that influence the transport of drugs into the body, which makes the drugs more concentrated and potentially dangerous. It can increase blood concentrations of some drugs by 1.5 to 15 times above normal for a particular dose...

It doens't interfere with absorbtion, it works on the the enzyme that is responsible for metabolism of many oral drugs - specificaly cytochrome P450 - iso-enzyme 3A4.
3A4 wil preferentially metabolize the grapfruit juice and therefore be 'tied up' and unavailable to metabolize other drugs. This won't ultimatley prevent the drugs metabolism, just slow it.
Most drugs are dosed at intervals which are based on the typical metabolism in the average person. Adjustments are often required in pateints that are renally or hepaticaly impared. Grapefruit juice has the potential to change the rate of metabolism of a drug, and when dosed at the normal interval, concentrations can build to toxic levels.
