Osteochondritis dissecans
The definition of Osteochondritis dissecans is when a piece of articular cartilage and a small layer of the underlying bone comes loose from the end of the bone. It happens most often in young men after an injury to the joint and while the knee is most often affected t can occur in the hip, shoulder, elbow and ankle. (I have have treated athletes that have had OD in the knee, elbow and ankle.)
OD is caused by a reduction in blood flow to the end of the bone on as a result of trauma. It is thought that their may be a genetic component.
The doc will probably try PT to reduce your symptoms. (At PT they will try to increase ROM, reduce swelling and strengthen the ankle.) The MD my also perform surgery to remove the loose bodies and perhaps even try to reattach the loose piece if it is large enough.

The anterior tilt issue is easily solved....more so then the ankle that is......with some vigorous stretching and reverse hyper type strengthening. You will need to work on the hip flexors standing, in prone and supine positions. If you don't have access to a reverse hyper you can also do RDLs, pull-throughs, regular hypers with a pause. Once you balance the length-tension relationship the anterior tilt should be corrected.

Mike,
I have a quick question about how to rehab my knee. I have "weakness" in the back of my knee. If i try to squat or even bend to get in a car, it feel like it can't hold my weight. It seems to affect my deadlift at higher weights and I can only do about 10 pounds on the leg extension. I only have pain on extensions. I have been trying doing these for rehab along with going light on deadlifts, not squatting and generally taking it easy. Any other suggestions?

1 other question I thought of, with your experience, is it possible/and or likely that a person can make a 100% full recovery with this "condition" anything I had read online sounds like my ankle may never be the same regardless of what treatment i have. Thanks again

I am a little confused. You say you have weakness in the "back of your knee". Do you mean in the hamstring? You also say you have pain with extensions. Do you mean leg extensions like on a Cybex?
The terminal knee extension exercise is for rehabilitation of a knee that is severely weak. Or a knee that is lacking full extension after a surgery. I would not use this particular exercise on a healthy athlete or an athlete that is just weak.

Thanks again for responses, appreciate the time, I met with a Physical Therapist today and she said where the "Defect" is located and how the missing body is floating around she said PT wouldn't do a whole lot, and she also mentioned that at this specific rate she wouldn't be surprised if that "chunk" of cartiladge went and got lodged into the joint and made surgery worse. So going to meet with a Specialist whenever I get time. Thanks again
